Injured Worker Packets & Educational Materials

MAI provides Employee Injury Packets that can be handed out to employees when they are injured. Each packet includes:

• A Policies & Procedures for Employees document
• An Ohio BWC First Report of Injury (FROI) form
• An Employee's Report of Injury form
• An Ohio BWC MEDCO-14 Physician's Report of Workability form
• An employee ID card that can be provided to the doctor when the employee goes for treatment
• If you have a company specific form you also use, then we can incorporate it into the packets

We periodically send out MAInside Info flyers to help educate or explain changes to the workers' compensation programs
